What Does Membership Include?

Membership provides access to a higher level of personalized care, including:

  • Same-day or next-day appointments whenever possible

  • 24/7 physician availability for urgent medical concerns—even when the office is closed

Will I Still Benefit If I'm Healthy?

Absolutely.

Many of our members join because they value prevention, convenience, and having a trusted physician who knows them well—not because they are frequently ill.

Our focus is helping patients stay healthy by identifying potential health concerns early, optimizing lifestyle habits, and developing individualized wellness plans before problems become serious.

Can I Reach My Physician After Hours?

Yes.

Members have access to a physician 24 hours a day, 7 days a week by calling our office at (617) 964-3333. After office hours, your call is connected directly to the physician on call.

Whether you have an urgent concern, need medical advice, or simply aren't sure whether something requires immediate attention, we're here when you need us.

What Happens If I'm Hospitalized?

Should you require hospitalization, your inpatient care will be managed by the hospital's physician team.

However, our physicians remain actively involved by closely reviewing your hospital treatment plan, coordinating your ongoing care, and assisting with your transition home. We also provide timely follow-up after discharge to ensure seamless continuity of care.

What If I Need a Specialist?

If specialty care is needed, we'll connect you with one of our trusted specialists based on your individual healthcare needs.

Our team coordinates referrals, appointments, testing, and communication with specialists to ensure your care remains organized and connected.

What If I Need Medical Care While Traveling?

If you're away from home and need medical advice, you can still contact our office.

Whenever medically appropriate, your physician can evaluate many concerns through a virtual visit and recommend treatment. If an in-person evaluation is necessary, we will recommend whether you should be seen at an urgent care center or the emergency room and coordinate any necessary follow-up upon your return.

What Should I Do If I Have a Medical Emergency?

If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency department immediately.

For urgent concerns that are not life-threatening, contact our office. In many cases, we can provide guidance over the phone or arrange for you to be seen the same day or the next business day, helping you avoid unnecessary visits to the emergency room or urgent care.

Do You Accept Insurance?

Yes.

We accept most major insurance plans and continue to bill your insurance for medical services, just as a traditional primary care practice would.

Do You Accept Medicare?

Yes.

We are participating providers with Medicare and will bill Medicare for covered medical services.

Many Medicare patients appreciate the additional time, accessibility, and care coordination our membership model provides, particularly when managing chronic medical conditions or multiple medications.

How Much Does Membership Cost?

Your annual membership fee is separate from insurance and provides enhanced access to your physician, longer appointments, comprehensive care coordination, and the personalized services that distinguish our practice.

Membership is available with annual or semiannual payment options.

  • Adults (27 years and older): $6,000 per year
  • Young Adults (ages 18–26): $3,000 per year (with a paying parent)

Can I Meet the Physicians Before Joining?

Yes.

We encourage all prospective members to schedule a complimentary Meet & Greet, either in person or virtually. This gives you the opportunity to learn more about our practice, ask questions, and determine whether Personalized Medical Care is the right fit for your healthcare needs.

How Do I Become a Member?

Our patient panels are currently full. Because we intentionally limit the size of our practice to maintain the highest standard of individualized care, membership availability is limited. Please check back with us in the future.
